Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000444.6(PHEX):c.2025_2034dup (p.Thr679fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the PHEX gene (transcript NM_000444.6) at coding-DNA position 2025 through coding-DNA position 2034, duplicating 10 bases; at the protein level this means shifts the reading frame starting at threonine residue 679, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the PHEX protein in which other variant(s) (p.Arg747*) have been determined to be pathogenic (PMID: 9199930, 9768674). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. This variant has not been reported in the literature in individuals affected with PHEX-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Thr679Hisfs*41) in the PHEX gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 71 amino acid(s) of the PHEX protein.
