Pathogenic for Landau-Kleffner syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001134407.3(GRIN2A):c.1285T>C (p.Cys429Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GRIN2A gene (transcript NM_001134407.3) at coding-DNA position 1285, where T is replaced by C; at the protein level this means replaces cysteine at residue 429 with arginine — a missense variant. Submitter rationale: This sequence change replaces cysteine, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 429 of the GRIN2A protein (p.Cys429Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of developmental and epileptic encephalopathy (internal data). In at least one individual the variant was observed to be de novo. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt GRIN2A protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic.
