Pathogenic — the classification assigned by GeneDx to NM_001164508.2(NEB):c.16620T>A (p.Tyr5540Ter), citing GeneDx Variant Classification (06012015). This variant lies in the NEB gene (transcript NM_001164508.2) at coding-DNA position 16620, where T is replaced by A; at the protein level this means converts the codon for tyrosine at residue 5540 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The Y5540X pathogenic variant in the NEB gene has not been reported previously as a pathogenic variant nor as abenign variant, to our knowledge. This variant is predicted to cause loss of normal protein function either throughprotein truncation or nonsense-mediated mRNA decay. The Y5540X variant was not observed in approximately 2300individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is nota common benign variant in these populations. We interpret Y5540X as a pathogenic variant.
