NM_000215.4(JAK3):c.2670C>G (p.Ser890Arg) was classified as Likely pathogenic for T-B+ severe combined immunodeficiency due to JAK3 deficiency by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the JAK3 gene (transcript NM_000215.4) at coding-DNA position 2670, where C is replaced by G; at the protein level this means replaces serine at residue 890 with arginine — a missense variant. Submitter rationale: This sequence change replaces serine, which is neutral and polar, with arginine, which is basic and polar, at codon 890 of the JAK3 protein (p.Ser890Arg). This variant is present in population databases (rs144195765, gnomAD 0.007%). This missense change has been observed in individual(s) with severe combined immunodeficiency (internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 2808244). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on JAK3 protein function. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
