This variant lies in the GALNS gene (transcript NM_000512.5) at coding-DNA position 2, where T is replaced by C; at the protein level this means replaces methionine at residue 1 with threonine — a missense variant. Submitter rationale: This sequence change affects the initiator methionine of the GALNS mRNA. The next in-frame methionine is located at codon 22. This variant is not present in population databases (gnomAD no frequency). Disruption of the initiator codon has been observed in individual(s) with mucopolysaccharidosis IVA (PMID: 15241807; Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. Studies have shown that disruption of the initiator codon alters GALNS gene expression (PMID: 15241807). For these reasons, this variant has been classified as Pathogenic.
