Likely pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_006231.4(POLE):c.890C>T (p.Ser297Phe), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces serine, which is neutral and polar, with phenylalanine, which is neutral and non-polar, at codon 297 of the POLE protein (p.Ser297Phe). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with autosomal dominant POLE-related conditions (PMID: 34816535). In at least one individual the variant was observed to be de novo. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt POLE protein function with a positive predictive value of 80%. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
