Likely pathogenic for Familial hemophagocytic lymphohistiocytosis 5 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_006949.4(STXBP2):c.247-2_284del, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the STXBP2 gene (transcript NM_006949.4) at the canonical splice acceptor site of the intron immediately before coding-DNA position 247 through coding-DNA position 284, deleting this region. Submitter rationale: In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. This variant has not been reported in the literature in individuals affected with STXBP2-related conditions. This variant is not present in population databases (gnomAD no frequency). This variant results in the deletion of part of exon 5 (c.247-2_284del) of the STXBP2 gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in STXBP2 are known to be pathogenic (PMID: 19804848, 22451424).
