NM_003073.5(SMARCB1):c.93+1G>A was classified as Likely pathogenic for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The c.93+1G>A intronic variant results from a G to A substitution one nucleotide after coding exon 1 of the SMARCB1 gene. This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. Loss-of-function variants in SMARCB1 are known to cause rhabdoid tumor predisposition syndrome; however, such associations with neurodevelopmental disorders are exceedingly rare (Kosho T et al. Am J Med Genet C Semin Med Genet. 2014 Sep;166C(3):262-75; Eaton KW et al. Pediatr Blood Cancer. 2011 Jan;56(1):7-15). Based on the supporting evidence, this variant is likely pathogenic for SMARCB1-related tumor predisposition syndromel; however, the association of this variant with Coffin-Siris syndrome is unlikely.
