This variant lies in the IL10RA gene (transcript NM_001558.4) at coding-DNA position 349, where C is replaced by T; at the protein level this means replaces arginine at residue 117 with cysteine — a missense variant. Submitter rationale: This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 117 of the IL10RA protein (p.Arg117Cys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with very early onset inflammatory bowel disease (PMID: 24001973, 30212871). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 2751753). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt IL10RA protein function with a positive predictive value of 80%. For these reasons, this variant has been classified as Pathogenic.
