Likely pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_206933.4(USH2A):c.12560G>C (p.Arg4187Pro), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with proline, which is neutral and non-polar, at codon 4187 of the USH2A protein (p.Arg4187Pro). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with deafness (Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt USH2A protein function. This variant disrupts the p.Arg4187 amino acid residue in USH2A. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 24938718, 31054281, 32050993, 32188678, 33090715; Invitae). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
