NM_000284.4(PDHA1):c.383G>A (p.Gly128Asp) was classified as Likely pathogenic for Pyruvate dehydrogenase E1-alpha deficiency by PDHA1 Study Group, University Children’s Hospital, Paracelsus Medical University. This variant lies in the PDHA1 gene (transcript NM_000284.4) at coding-DNA position 383, where G is replaced by A; at the protein level this means replaces glycine at residue 128 with aspartic acid — a missense variant. Submitter rationale: The NM_000284.3:c.383G>A (p.Gly128Asp) substitution is a missense variant in PDHA1 gene.In total, 2 individuals were diagnosed with PDHA1-related Pyruvate dehydrogenase complex (PDHc) deficiency (MIM #312170). These include 1 male and 1 female. Among them, 1 were confirmed inherited. The variant has been reported in 1 published case (PMIDs: 28918066, 23021068). Additional 1 unpublished case from internal data is included. Last literature search: July 12, 2024. This variant is absent or extremely rare in population-based cohorts in the Genome Aggregation Database (gnomAD). Individuals harboring this variant presented with clinical features compatible with PDHA1-related PDHc deficiency. In summary, this variant meets criteria to be classified as likely pathogenic (LP) for PDHA1-related PDHc deficiency based on the ACMG/AMP criteria applied: PM1, PM2, PM7, PP3 (last assessment October 15, 2024).
