Likely pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001287.6(CLCN7):c.896C>T (p.Ala299Val), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces alanine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 299 of the CLCN7 protein (p.Ala299Val). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with autosomal recessive osteopetrosis (PMID: 26477479, 33125761). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant has been reported in individual(s) with autosomal dominant osteopetrosis (PMID: 23953223); however, the role of the variant in this condition is currently unclear. An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects CLCN7 function (PMID: 35159175). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
