Pathogenic for Purine-nucleoside phosphorylase deficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000270.4(PNP):c.569G>T (p.Gly190Val), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glycine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 190 of the PNP protein (p.Gly190Val). This variant is present in population databases (no rsID available, gnomAD 0.003%). This missense change has been observed in individual(s) with clinical features of purine nucleoside phosphorylase deficiency and/or purine nucleoside phosphorylase deficiency (PMID: 9067751, 22132981, 38431953; internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 2736067). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on PNP protein function. For these reasons, this variant has been classified as Pathogenic.
