Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000039.3(APOA1):c.494T>G (p.Leu165Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces leucine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 165 of the APOA1 protein (p.Leu165Arg). This variant is present in population databases (rs779081481, gnomAD 0.0009%). This missense change has been observed in individual(s) with APOA1 deficiency (PMID: 8840853). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. This variant is also known as (L141R) Pisa. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt APOA1 protein function with a positive predictive value of 80%. Experimental studies have shown that this missense change affects APOA1 function (PMID: 26363436). For these reasons, this variant has been classified as Pathogenic.
