This variant lies in the GCK gene (transcript NM_000162.5) at coding-DNA position 637, where T is replaced by C; at the protein level this means replaces cysteine at residue 213 with arginine — a missense variant. Submitter rationale: This sequence change replaces cysteine, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 213 of the GCK protein (p.Cys213Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with maturity-onset diabetes of the young (PMID: 9049484, 17937063, 19790256). It has also been observed to segregate with disease in related individuals. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt GCK protein function with a negative predictive value of 80%.
