Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001024630.4(RUNX2):c.560T>C (p.Phe187Ser), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the RUNX2 gene (transcript NM_001024630.4) at coding-DNA position 560, where T is replaced by C; at the protein level this means replaces phenylalanine at residue 187 with serine — a missense variant. Submitter rationale: This sequence change replaces phenylalanine, which is neutral and non-polar, with serine, which is neutral and polar, at codon 187 of the RUNX2 protein (p.Phe187Ser). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of cleidocranial dysplasia (PMID: 20648631; Invitae). In at least one individual the variant was observed to be de novo. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt RUNX2 protein function with a positive predictive value of 80%. For these reasons, this variant has been classified as Pathogenic.
