Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_183357.3(ADCY5):c.1313G>C (p.Arg438Pro), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with proline, which is neutral and non-polar, at codon 438 of the ADCY5 protein (p.Arg438Pro). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with autosomal dominant ADCY5-related conditions (PMID: 26686870). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ADCY5 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
