NM_001165963.4(SCN1A):c.4507G>A (p.Glu1503Lys) was classified as Pathogenic for Early-infantile DEE by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the SCN1A gene (transcript NM_001165963.4) at coding-DNA position 4507, where G is replaced by A; at the protein level this means replaces glutamic acid at residue 1503 with lysine — a missense variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt SCN1A protein function. This missense change has been observed in individual(s) with borderline severe myoclonic epilepsy (SMEB) and seizures (PMID: 19783390, 23195492; Invitae). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces glutamic acid, which is acidic and polar, with lysine, which is basic and polar, at codon 1503 of the SCN1A protein (p.Glu1503Lys).
