Pathogenic for Deficiency of hydroxymethylglutaryl-CoA lyase — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000191.3(HMGCL):c.144G>T (p.Lys48Asn), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces lysine, which is basic and polar, with asparagine, which is neutral and polar, at codon 48 of the HMGCL protein (p.Lys48Asn). RNA analysis indicates that this missense change induces altered splicing and likely results in a shortened protein product. This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with HMG-CoA lyase deficiency (PMID: 17459752, 19177531, 34573903). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects HMGCL function (PMID: 17459752). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that this missense change results in skipping of exon 2, but is expected to preserve the integrity of the reading-frame (PMID: 23465862). For these reasons, this variant has been classified as Pathogenic.
