This variant lies in the NMNAT1 gene (transcript NM_022787.4) at coding-DNA position 721, where C is replaced by T; at the protein level this means replaces proline at residue 241 with serine — a missense variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt NMNAT1 protein function. This missense change has been observed in individual(s) with Leber congenital amaurosis (PMID: 25988908, 26047050, 31630094, 34837036). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces proline, which is neutral and non-polar, with serine, which is neutral and polar, at codon 241 of the NMNAT1 protein (p.Pro241Ser).
