Uncertain significance for Birt-Hogg-Dube syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_144997.7(FLCN):c.65C>G (p.Thr22Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces threonine, which is neutral and polar, with arginine, which is basic and polar, at codon 22 of the FLCN protein (p.Thr22Arg). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with FLCN-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt FLCN protein function with a positive predictive value of 80%. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site.
