Pathogenic for Early-infantile DEE — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001165963.4(SCN1A):c.4852G>C (p.Gly1618Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glycine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 1618 of the SCN1A protein (p.Gly1618Arg). This variant also falls at the last nucleotide of exon 25, which is part of the consensus splice site for this exon. This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of autosomal dominant developmental and epileptic encephalopathy (Invitae). In at least one individual the variant was observed to be de novo. An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. This variant disrupts the c.4852G nucleotide in the SCN1A gene. Other variant(s) that disrupt this nucleotide have been determined to be pathogenic (PMID: 34938262). This suggests that this nucleotide is clinically significant, and that variants that disrupt this position are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
