NM_007194.4(CHEK2):c.300G>T (p.Gln100His) was classified as Uncertain significance for CHEK2-related cancer predisposition by Department of Genetics, HCU Lozano Blesa: Variant summary: CHEK2 c.300G>T results in the replacement of Gln100 by a His residue (p.Gln100His). The variant was identified in 1 out of 396 patients analysed (freq: 0.0025, doi: 10.3389/fgene.2023.1274108). The patient was a woman that developed breast cancer at the age of 54 (luminal B tumor phenotype). One first-degree relative of her was also diagnosed with BC, although a co-segregation study could not be performed. Gln100 is located in the FHA domain, is exposed to solvent, forms a hydrogen bond (H-bond) with Asn196, and is far (>10 Å) from any atom of the opposite FHA domain (i.e. from the dimerization surface) and from the catalytic center of CHEK2 protein. Its replacement by histidine at this position would imply the introduction of a positive charge. Our in-silico study on the protein stability based on relaxation molecular dynamics simulations (doi: 10.3389/fgene.2023.1274108) indicates that the variant is unlikely to induce structural destabilization. The variant is reported in gnomAD v4 (freq=1.6x10-4 %; 1 case in 628626 alleles). Only one submitter have assessed this variant in ClinVar (as of Uncertain Significance), while other replacements described at this position (Q100P, Q100R, and Q100K) appear classified in this database as of Uncertain Significance. The sequence conservation of the position is moderated (Gln100 is the second residue with the highest prevalence, where Gly predominates and Ala is also found with moderate conservation among CHEK2 similar sequences). The metapredictor PirePred (relies on verdicts from other 15 predictors or metapredictors), the AI-based predictor AlphaMissense, as well as the ACMG classification tool Franklin classify the variant as of Uncertain Significance. Since supporting evidence is limited at this time, the clinical significance of this variant remains unclear.
