This variant lies in the DARS2 gene (transcript NM_018122.5) at coding-DNA position 1440, where G is replaced by A; at the protein level this means converts the codon for tryptophan at residue 480 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The p.Trp480Ter variant in DARS2 has not been previously reported in the literature in individuals with leukoencephalopathy with brain stem and spinal cord involvement-high lactate syndrome, but has been identified in 0.002% (1/59992) of Admixed American chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP rs139747670). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (Variation ID: 2682779) and has been interpreted as likely pathogenic by Mayo Clinic Laboratories (Mayo Clinic). This nonsense variant leads to a premature termination codon at position 480, which is predicted to lead to a truncated or absent protein. Loss of function of the DARS2 gene is an established disease mechanism in leukoencephalopathy with brain stem and spinal cord involvement-high lactate syndrome. In summary, although additional studies are required to fully establish its clinical significance, this variant is likely pathogenic for autosomal recessive leukoencephalopathy with brain stem and spinal cord involvement-high lactate syndrome.
