This variant lies in the POLG gene (transcript NM_002693.3) at coding-DNA position 3626 through coding-DNA position 3629, duplicating 4 bases; at the protein level this means converts the codon for tyrosine at residue 1210 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: This sequence change creates a premature translational stop signal (p.Tyr1210*) in the POLG gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 30 amino acid(s) of the POLG protein. This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with autosomal recessive POLG-related conditions (PMID: 20691285, 30385167, 30423451). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 2677952). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. For these reasons, this variant has been classified as Pathogenic.
