This variant lies in the NR5A1 gene (transcript NM_004959.5) at coding-DNA position 274, where C is replaced by T; at the protein level this means replaces arginine at residue 92 with tryptophan — a missense variant. Submitter rationale: This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 92 of the NR5A1 protein (p.Arg92Trp). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with NR5A1-related conditions (PMID: 27378692, 27490115, 28033660). In at least one individual the variant was observed to be de novo. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 265792). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt NR5A1 protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects NR5A1 function (PMID: 27378692). For these reasons, this variant has been classified as Pathogenic.
