NM_000444.6(PHEX):c.1363G>T (p.Glu455Ter) was classified as Pathogenic by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the PHEX gene (transcript NM_000444.6) at coding-DNA position 1363, where G is replaced by T; at the protein level this means converts the codon for glutamic acid at residue 455 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The E455X nonsense variant in the PHEX gene has been reported previously in association with X-linked hypophosphatemic rickets (Filisetti et al., 1999; Song et al., 2007; Lee et al., 2012). This pathogenic variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. Therefore, we consider this variant to be pathogenic.
