This variant lies in the L1CAM gene (transcript NM_001278116.2) at coding-DNA position 1453, where C is replaced by T; at the protein level this means converts the codon for arginine at residue 485 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The R485X nonsense variant in the L1CAM gene has been reported previously in association with X-linked hydrocephalus (Jouet et al., 1994; Adle-Biassette et al., 2013). R485X is not observed in large population cohorts (Lek et al., 2016). This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. We interpret this as a pathogenic variant.
