Pathogenic — the classification assigned by GeneDx to NM_000512.5(GALNS):c.1485C>G (p.Asn495Lys), citing GeneDx Variant Classification (06012015). This variant lies in the GALNS gene (transcript NM_000512.5) at coding-DNA position 1485, where C is replaced by G; at the protein level this means replaces asparagine at residue 495 with lysine — a missense variant. Submitter rationale: The N495K variant in the GALNS gene has been reported previously in the compound heterozygous state in three individuals with mucopolysaccharidosis IVA (Dung et al., 2013; Caciotti et al., 2015). The N495K variant was not observed in approximately 6400 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The N495K variant is a semi-conservative amino acid substitution, which may impact secondary protein structure as these residues differ in some properties. This substitution occurs at a position that is conserved across species. In silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. A missense variant in the same residue (N495Y) has been reported in a Spanish individual with mucopolysaccharidosis IVA (Pajares et al., 2012), and missense variants in nearby residues (A492T, M494V, P498L, P499L, G500S) have been reported in the Human Gene Mutation Database in association with mucopolysaccharidosis IVA (Stenson et al., 2014), supporting the functional importance of this region of the protein. We interpret N495K as a pathogenic variant.
