NM_053025.4(MYLK):c.4336G>A (p.Glu1446Lys) was classified as Uncertain significance by Women's Health and Genetics/Laboratory Corporation of America, LabCorp, citing LabCorp Variant Classification Summary - May 2015. This variant lies in the MYLK gene (transcript NM_053025.4) at coding-DNA position 4336, where G is replaced by A; at the protein level this means replaces glutamic acid at residue 1446 with lysine — a missense variant. Submitter rationale: Variant summary: MYLK c.4336G>A (p.Glu1446Lys) results in a conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 0.00011 in 251468 control chromosomes. The observed variant frequency is approximately 2 fold of the estimated maximal expected allele frequency for a pathogenic variant in MYLK causing Thoracic Aortic Aneurysms And Dissections phenotype (5e-05), strongly suggesting that the variant is benign. To our knowledge, no occurrence of c.4336G>A in individuals affected with Thoracic Aortic Aneurysms And Dissections has been reported. At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect results in >50%-90% of normal activity (Kwartler_2018). Five clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as VUS-possibly benign.
