Submitter rationale: The c.4126dupG pathogenic mutation, located in coding exon 28 of the SMARCA4 gene, results from a duplication of G at nucleotide position 4126, causing a translational frameshift with a predicted alternate stop codon (p.E1376Gfs*24). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. Loss-of-function variants in SMARCA4 are known to cause rhabdoid tumor predisposition syndrome including small cell carcinoma of the ovary-hypercalcemic type (SCCOHT); however, such associations with neurodevelopmental disorders are exceedingly rare (Kosho T et al.
