Uncertain Significance for Classic or attenuated familial adenomatous polyposis — the classification assigned by All of Us Research Program, National Institutes of Health to NM_000038.6(APC):c.8216_8231del (p.Lys2739fs), citing ACMG Guidelines, 2015: This variant deletes 16 nucleotides in exon 16 of the APC gene, creating a frameshift and premature translation stop signal. This mutant transcript is predicted to escape nonsense-mediated decay and be expressed as a truncated protein. Although functional studies have not been reported, this variant is expected to disrupt part of EB1 and HDLG binding sites, which mediate interactions with the cytoskeleton (PMID: 15311282, 17293347). To our knowledge, this variant has not been reported in individuals affected with APC-related disorders in the literature. This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Loss of APC function is a known mechanism of disease (clinicalgenome.org). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
