Uncertain significance — the classification assigned by Women's Health and Genetics/Laboratory Corporation of America, LabCorp to NM_000181.4(GUSB):c.1618G>A (p.Glu540Lys), citing LabCorp Variant Classification Summary - May 2015: Variant summary: GUSB c.1618G>A (p.Glu540Lys) results in a conservative amino acid change located in the Glycoside hydrolase family 2, catalytic domain (IPR006103) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251100 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1618G>A has been reported in the literature in at least one individual affected with Mucopolysaccharidosis Type VII (Sly Syndrome) (Tomatsu_2002, Tomatsu_2009). These data do not allow any conclusion about variant significance. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. In addition, recombinant E540A human GUS had no catalytic activity, but the E540Q GUS showed 0.3% of WT activity, supporting this residue (E540) is important to enzyme activity. The following publications have been ascertained in the context of this evaluation (PMID: 26415878, 19224584, 12403825, 10438523). No submitters have cited clinical-significance assessments for this variant to ClinVar after 2014. Based on the evidence outlined above, the variant was classified as uncertain significance.
