Likely pathogenic for Retinitis pigmentosa 25 — the classification assigned by Broad Center for Mendelian Genomics, Broad Institute of MIT and Harvard to GRCh38/hg38 6q12(chr6:65331371-65405465)x1, citing ACMG/ClinGen CNV Guidelines, 2019. This is a single-copy loss (one copy instead of two) of the chr6:65331371-65405465 region (~74.1 kb) on cytogenetic band 6q12. Submitter rationale: A heterozygous deletion of exons 5-11 in EYS (NM_001142800.2) was identified by exome sequencing in one individual with retinitis pigmentosa in the compound heterozygous state, along with a known likely pathogenic variant (Variation ID: 194358) ([GRCh 38] chr6:65331371_65405465x1)(PMID: 34906470). These breakpoints have been estimated by exome sequencing only and therefore may not reflect the true breakpoints. Inheritance information is unavailable. The patient phenotype is nonspecific, but is consistent with cases described in the literature and/or published databases with overlapping variants. This intragenic variant is predicted to cause a frameshift, which alters the protein’s amino acid sequence beginning at exon 5 and leads to a premature termination codon. This alteration is then predicted to lead to a truncated or absent protein. Loss of function of EYS is an established disease mechanism in autosomal recessive retinitis pigmentosa (https://search.clinicalgenome.org/kb/gene-dosage). Two deletions overlapping this exon have been identified in the Genome Aggregation Database: DEL_6_69850 in 1/1930(0.05%) Latino chromosomes and in 1/2416 (0.04%) of East Asian chromosomes, and DEL_6_69861 in 1/1930 (0.05%) of Latino chromosomes (gnomAD, http://gnomad.broadinstitute.org; Structural variant: DEL_6_69850, DEL_6_69861), with no homozygotes. In summary, although additional studies are required to fully establish its clinical significance, this variant is likely pathogenic for autosomal recessive retinitis pigmentosa.
