GRCh38/hg38 4p16.3-13(chr4:2904667-42963232)x3 was classified as Pathogenic for Neurodevelopmental disorder by Broad Center for Mendelian Genomics, Broad Institute of MIT and Harvard, citing ACMG/ClinGen CNV Guidelines, 2019. This is a single-copy gain (three copies) of the chr4:2904667-42963232 region (~40.06 Mb) on cytogenetic band 4p16.3-13. Submitter rationale: A confirmed de novo heterozygous duplication of 4p16.3-p13 encompassing 127 genes (https://genescout.omim.org/) was identified by exome sequencing in one individual with agenesis of corpus callosum ([GRCh38] chr4:2904667_42963232x3). These breakpoints have been estimated by exome sequencing only and therefore may not reflect the true breakpoints. The patient phenotype is nonspecific, but is consistent with cases described in the literature and/or published databases with overlapping variants. There are no known triplosensitive genes contained within the duplicated region, but a triplosensitivity predictor suggests that multiple genes (PHOX2B, HTT, RBPJ, PDS5A, RGS12) included in this duplication are triplosensitive (https://www.deciphergenomics.org/). In summary, the 4p16.3-p13 duplication meets criteria to be classified as pathogenic.
