Likely pathogenic for Walker-Warburg congenital muscular dystrophy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_024301.5(FKRP):c.469G>C (p.Ala157Pro), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces alanine, which is neutral and non-polar, with proline, which is neutral and non-polar, at codon 157 of the FKRP protein (p.Ala157Pro). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with dystroglycanopathy and/or limb girdle muscular dystrophy (PMID: 16634037, 21970816, 29101272; internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 2577157). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on FKRP protein function. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
