This variant lies in the ABCC8 gene (transcript NM_000352.6) at coding-DNA position 4259, where G is replaced by A; at the protein level this means replaces arginine at residue 1420 with histidine — a missense variant. Submitter rationale: The p.Arg1420His variant in ABCC8 has been reported in 5 individuals with hyperinsulinemic hypoglycemia (PMID: 26246406, 25720052, 35182381, 23926410, 15579781), and has been identified in 0.015% (4/25886) of Latino/Admixed American chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP rs1272388614). Although this variant has been seen in the general population in a heterozygous state, its frequency is not high enough to rule out a pathogenic role. Of the 5 affected individuals, 2 were compound heterozygotes that carried a reported pathogenic variant in trans, which increases the likelihood that the p.Arg1420His variant is pathogenic (PMID: 26246406, 25720052, 23926410, 35182381). Computational prediction tools and conservation analyses suggest that this variant may impact the protein, though this information is not predictive enough to determine pathogenicity. In vitro functional studies provide some evidence that the p.Arg1420His variant may slightly impact protein function (PMID: 25720052, 15579781). However, these types of assays may not accurately represent biological function.
