This variant lies in the SFTPC gene (transcript NM_001317778.2) at coding-DNA position 134, where T is replaced by C; at the protein level this means replaces leucine at residue 45 with proline — a missense variant. Submitter rationale: This SFTPC missense variant is absent from a large population dataset and has not been reported in ClinVar nor the literature, to our knowledge. A different variant at the same nucleotide position (c.134T>G;p.Leu45Arg) has been reported in individuals that had findings consistent with pulmonary surfactant metabolism dysfunction. Two bioinformatic tools queried predict that c.134T>C in SFTPC would be damaging, but these algorithms have low specificity, especially for predicting gain of function or dominant negative variants. The leucine residue at this position is evolutionarily conserved across most of the species assessed. We consider the clinical significance of c.134T>C;p.Leu45Pro in SFTPC to be uncertain at this time.
