Likely Pathogenic for Hypotonia, infantile, with psychomotor retardation and characteristic facies 3 — the classification assigned by Broad Center for Mendelian Genomics, Broad Institute of MIT and Harvard to NM_001163435.3(TBCK):c.468C>G (p.Tyr156Ter), citing ACMG Guidelines, 2015: The p.Tyr156Ter variant in TBCK has not been previously reported in the literature in individuals with TBCK-related intellectual disability syndrome, but has been identified in 0.0002% (2/1176490) of European (non-Finnish) chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (Variation ID: 2574702) and has been interpreted as likely pathogenic by Institute of Human Genetics, University of Goettingen. This nonsense variant leads to a premature termination codon at position 156, which is predicted to lead to a truncated or absent protein. Loss of function of the TBCK gene is an established disease mechanism in autosomal recessive TBCK-related intellectual disability syndrome. In summary, although additional studies are required to fully establish its clinical significance, this variant is likely pathogenic for autosomal recessive TBCK-related intellectual disability syndrome.
