This variant lies in the SLC22A5 gene (transcript NM_003060.4) at coding-DNA position 428, where C is replaced by A; at the protein level this means replaces proline at residue 143 with glutamine — a missense variant. Submitter rationale: Variant summary: SLC22A5 c.428C>A (p.Pro143Gln) results in a non-conservative amino acid change located in the Major facilitator superfamily domain (IPR020846) of the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251496 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.428C>A in individuals affected with Systemic Primary Carnitine Deficiency and no experimental evidence demonstrating its impact on protein function have been reported. However, a different missense variant affecting the same amino acid (Pro143Leu), is classified as pathogenic by our laboratory, indicating that this residue might be functionally important. No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Based on the evidence outlined above, the variant was classified as uncertain significance.
