Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000314.8(PTEN):c.138C>G (p.Tyr46Ter), citing Ambry Variant Classification Scheme 2023: The p.Y46* pathogenic mutation (also known as c.138C>G), located in coding exon 2 of the PTEN gene, results from a C to G substitution at nucleotide position 138. This changes the amino acid from a tyrosine to a stop codon within coding exon 2. This variant demonstrated low intracellular protein abundance on one multiplex functional assay (Matreyek KA et al. Nat Genet, 2018 Jun;50:874-882). In a massively parallel functional assay using a humanized yeast model, lipid phosphatase activity for this variant was functionally deficient (Mighell TL et al. Am J Hum Genet, 2018 May;102:943-955). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
