This variant lies in the LDLR gene (transcript NM_000527.5) at the canonical splice donor site of the intron immediately after coding-DNA position 940, where T is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.940+2T>A intronic variant results from a T to A substitution two nucleotides after coding exon 6 in the LDLR gene. Alterations that disrupt the canonical splice site are expected to result in aberrant splicing. In silico splice site analysis predicts that this alteration will weaken the native splice donor site and may result in the creation or strengthening of a novel splice donor site. A resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNAdecay; although, direct evidence is unavailable. However, the region predicted to be impacted is critical for protein function (Ambry internal data). This variant was reported in individual(s) with features consistent with familial hypercholesterolemia (FH) (Marduel M et al. Hum Mutat, 2010 Nov;31:E1811-24; Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species.
