NM_001384125.1(BLTP1):c.4719G>A (p.Met1573Ile) was classified as Uncertain significance by Women's Health and Genetics/Laboratory Corporation of America, LabCorp, citing LabCorp Variant Classification Summary - May 2015: Variant summary: KIAA1109 (aka. BLTP1) c.4719G>A (p.Met1573Ile) results in a conservative amino acid change in the encoded protein sequence. Three of four in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 249240 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.4719G>A has been reported in the literature in a compound heterozygous individual who carried another missense variant in trans, and had symptoms suggestive of Alkuraya-Kucinskas Syndrome (Gueneau_2017). Authors of this study noted that all their cases compatible with life (i.e. three surviving patients from two families) carried biallelic missense variants, consistent with the causative role for hypomorphic variants in this syndrome, and a later study (describing four additional surviving patients from two related families who carried a homozygous missense variant) was in agreement with this rationale (Kumar_2020). However, these data do not allow clear conclusions about variant significance. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 29290337, 32590954). No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic.
