Likely pathogenic for Paroxysmal nocturnal hemoglobinuria 1 — the classification assigned by Pangenia Genomics, Pangenia Inc. to NM_002641.4(PIGA):c.1139del (p.Ile380fs): This variant change creates a premature translational stop signal (p.Ile380Lysfs*2) in the PIGA gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt >10% of the PIGA protein at the C-terminus. Other variant(s) that disrupt this region (p.Arg412*) have been determined to be pathogenic (PMID: 22305531, 24706016, 26545172). Another variant (c.1323_1324del, p.Leu442fs) leading to a stop signal further downstream has been described in a PNH patient (PubMed: 8167330). This suggests that variants that disrupt this region of the protein are likely to disrupt protein function. This variant is absent from the gnomAD v2.1.1 dataset with good coverage of the locus.
