Uncertain significance — the classification assigned by GeneDx to NM_005431.2(XRCC2):c.596T>A (p.Met199Lys), citing GeneDx Variant Classification (06012015): This variant is denoted XRCC2 c.596T>A at the cDNA level, p.Met199Lys (M199K) at the protein level, and results in the change of a Methionine to a Lysine (ATG>AAG). This variant has not, to our knowledge, been published in the literature as pathogenic or benign. XRCC2 Met199Lys was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, suggesting it is not a common benign variant in these populations. Since Methionine and Lysine differ in polarity, charge, size or other properties, this is considered a non-conservative amino acid substitution. XRCC2 Met199Lys occurs at a position where amino acids with properties similar to Methionine are tolerated across species and is located within the ATPase domain (Kim 2011). In silico analyses are inconsistent regarding the effect this variant may have on protein structure and function. Based on currently available evidence, it is unclear whether XRCC2 Met199Lys is a pathogenic or benign variant. We consider it to be a variant of uncertain significance.
