NM_144573.4(NEXN):c.1400del (p.Ile467fs) was classified as Uncertain significance for Cardiovascular phenotype by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The c.1400delT variant, located in coding exon 10 of the NEXN gene, results from a deletion of one nucleotide at nucleotide position 1400, causing a translational frameshift with a predicted alternate stop codon (p.I467Kfs*45). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. Although biallelic loss of function alterations in NEXN have been associated with autosomal recessive NEXN-related cardiomyopathy, haploinsufficiency for NEXN has not been clearly established as a mechanism of disease for autosomal dominant NEXN-related cardiomyopathy. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
