NM_004415.4(DSP):c.4609C>A (p.Arg1537Ser) was classified as Uncertain significance for Cardiovascular phenotype by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the DSP gene (transcript NM_004415.4) at coding-DNA position 4609, where C is replaced by A; at the protein level this means replaces arginine at residue 1537 with serine — a missense variant. Submitter rationale: The p.R1537S variant (also known as c.4609C>A), located in coding exon 23 of the DSP gene, results from a C to A substitution at nucleotide position 4609. The arginine at codon 1537 is replaced by serine, an amino acid with dissimilar properties. This alteration has been reported in a sudden unexplained death in infancy cohort and a limb-girdle muscular dystrophy cohort (Hertz CL et al. Eur J Hum Genet, 2016 06;24:817-22; Fichna JP et al. Hum Genomics, 2018 07;12:34). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
