Uncertain significance for Maple syrup urine disease, mild variant; Elevated circulating L-alloisoleucine concentration — the classification assigned by Institute of Human Genetics, University of Goettingen to NM_005881.4(BCKDK):c.486C>A (p.His162Gln), citing ACMG Guidelines, 2015: The variant c.486C>A (p.(His162Gln)) in exon 6 of the BCKDK-gene is not found in the gnomAD database, it affects a weakly conserved nucleotide, a highly conserved amino acid within a protein domain and there is a small physicochemical difference between His and Gln. This variant is described in the literature and was identified in an individual with a mild form of MSUD and in his clinically unaffected father, who had only slightly elevated BCAAs. The authors proposed a dominant-negative effect of this variant (PMID: 35205278). Variant was found to be de novo in our patient with biochemical tests confirming a MSUD in our patient.
