Likely pathogenic — the classification assigned by Genetic Services Laboratory, University of Chicago to NM_001109809.5(ZFP57):c.722dup (p.Cys241fs): DNA sequence analysis of the ZFP57 gene demonstrated a single base pair duplication in exon 5, c.722dup. This sequence change results in an amino acid frameshift and creates a premature stop codon an amino acid downstream of the change, p.Cys241Trpfs*2. This sequence change is predicted to result in an abnormal transcript, which may be degraded, or may lead to the production of a truncated ZFP57 protein with potentially abnormal function. p.Cys241Trpfs*2. While this duplication has not previously been described in the literature, other truncating and frameshift variants in the ZFP57 gene have been described in several individuals with ZFP57-related transient neonatal diabetes mellitus (PMIDs: 18622393, 25755231). The c.722dup sequence change has not been described in population databases such as ExAC and gnomAD. These collective evidences indicate that this sequence change is likely pathogenic, however functional studies have not been performed to prove this conclusively.
