Likely pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_020247.5(COQ8A):c.902G>A (p.Arg301Gln), citing Invitae Variant Classification Sherloc (09022015): In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. This variant disrupts the p.Arg301 amino acid residue in COQ8A. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 29915382, 30637285, 31890231, 32337771). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. This missense change has been observed in individual(s) with primary coenzyme Q10 deficiency (PMID: 32743982). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 301 of the COQ8A protein (p.Arg301Gln).
